(1) Tribal Housing staff will monitor each client’s service plan and each client household’s need for assistance. Tribal Housing staff will address continuation of services on a case-by-case basis, if initial service was provided by a third party.

(2) A formal termination-of-service evaluation and assessment will be conducted to address the services provided under the service plan and whether there are other unmet needs in the household that are preventing the client from stabilizing their housing. In households with unmet needs, Tribal Housing staff and household will work together to develop an aftercare plan, and follow up with a revised service plan when possible and appropriate.
