Information requirements common to most grants and contract applications include:

(1) Full name, address, and telephone number of the organization applying for the grant or contract. Do not use department or program name. Use the address below.

(2) Documentation of the organization’s authority to request a grant or contract (i.e., Tribal Council resolution).

(3) Date of submission to the Federal and State agency and the name of the office to which submitted.

(4) Signature of the Tribal Chairman or designee, and the date signed.

(5) The application for funding should be completed and submitted with any required attachments per the funding agency’s request.

(6) A detailed budget using the approved Samish budget form showing the amount of funds supporting the grant or contract.

(7) Advance payment or award letter is required by the Nation to perform the grant or contract. Any grants or contracts for which advance payment or award letters are not received shall have activity stopped until such a time that an advance payment or award letter is received.
