(1) Generally. Employees are required to report all injuries, illnesses, and incidents that occur on duty, as well as off-duty injuries, illnesses, and incidents that impact the employee’s ability to perform the duties and responsibilities at work.

(2) On-Duty Injuries or Illnesses.

(a) Employees who become ill on the job or suffer any work-related injury, no matter how minor, are to report to Senior Human Resources for assessment and recording of the incident. Employees may be required to seek immediate medical attention for injuries or illnesses that may fall under the protection of workers’ compensation. Please refer to STP 5-05-004 regarding workers’ compensation.

(b) Employees are required to report all vehicular incidents or accidents involving Tribal vehicles immediately, including all accidents, tickets, and violations. An “Accident Reporting Kit” is located in the glove compartment of each vehicle and must be completed and submitted to Senior Human Resources within 24 hours.

(3) Off-Duty Injuries or Illnesses. Employees who suffer any injuries or illnesses while off-duty must report the injury to Senior Human Resources as soon as possible if it impacts the employee’s ability to perform their regular work satisfactorily without endangering themselves or their fellow employees. Employees must provide a doctor’s note detailing any limitations and estimated time frame. Employees must also provide a doctor’s note to return to regular duty.

(4) Incidents. Employees are required to report any incidents that impact their ability to perform their regular job duties, work their regularly scheduled hours, or have any other adverse effect on the employee’s ability to perform all job duties and requirements as outlined in the job description. This includes but is not limited to: ability or eligibility to drive Tribal vehicles or criminal convictions.

(5) Changes That Affect Benefits. Employees are required to report changes that affect their employee benefits. Such changes include but are not limited to a domestic relations order if an employee is a 401K participant. [Res. 2022-10-016 § 1, 2022.]
