(1) All eligible applicants must complete a Samish Nation K – 12 education supplemental assistance application that includes the following documents:

(a) A completed Samish K – 12 education supplemental assistance application which includes:

(i) For K – 12 school students:

(A) Letter from classroom teacher explaining need; and/or

(B) Copy of individual education plan (IEP) or equivalent summary that outlines subjects where tutoring is needed;

(C) Contact information for desired tutor/service provider;

(D) Projected schedule of services requested;

(E) Cost estimate for requested service assistance.

(ii) For GED applications:

(A) Contact information for testing/prep program/service.

(B) Projected schedule for testing.

(C) Cost estimate for requested service assistance.

(2) Depending on the situation and the application submitted, additional documentation may be required in order for the request to be considered.

(3) First-time applicants must submit a complete application packet with all required support materials to be eligible for a funding determination.

(4) Renewing or returning applicants must resubmit information that is time sensitive (e.g., report cards, letter from teachers, etc.).

(5) All applicants (first time or renewing) must apply for supplemental funding on an academic year basis with any required supporting documentation requested by program staff.

(6) Based on funding availability and timing of applications, renewal of funds is not guaranteed.

(7) The timeline for ensuring that the application is considered in a timely manner is listed below:

(a) Application deadlines for all new and returning applicants are open and ongoing. All complete applications are reviewed as they are received on a first-come, first-served basis.
