(a) The name of the applicant;

(b) If the applicant is a corporation, the date of its incorporation, the period of its duration, and the laws under which it was incorporated (tribal, state, or federal);

(c) The Samish Indian Nation enrollment number of the applicant, if applicable, or if the applicant is a tribally owned corporation, the federally recognized tribe under the laws of which the corporation was formed; provided, however, that if the applicant is a tribally licensed corporation of a federally recognized tribe other than the Samish Indian Nation, the applicant must also provide an authenticated copy of its designation by the Samish Tribal Council as an agent of the Tribe;

(d) The address of the applicant including where the applicant is a corporation, its principal place of business and the names and respective addresses of its directors and officers;

(e) A statement that the applicant and its directors, officers, employees, counsel or consultants, contractors, and agents expressly consent to the jurisdiction of the Samish Tribal Court for all matters arising from business in Samish Indian Country;

(f) That the applicant irrevocably consents to the service of process upon it in accordance with Samish Indian Nation tribal law;

(g) If the applicant is a corporation, a statement that the officers executing the application have been duly authorized by the board of directors of the corporation; and

(h) Signature of and acknowledgement by the applicant, including where the applicant is a corporation, by its president or vice president and by its secretary or an assistant secretary.
